Without retention, relapse is virtually guaranteed as the periodontal fibers and bone need time to remodel around the new tooth positions (typically 12+ months of stability). Retention protocols vary but typically include: fixed retainers (thin wire bonded behind front teeth, can be permanent) for lower teeth, removable retainers (Essix clear or Hawley wire) for upper teeth, and full-time wear initially (22+ hours/day for 6-12 months) transitioning to nighttime-only wear. Most orthodontists now recommend lifelong nighttime retention. Additional aftercare includes maintaining excellent hygiene around fixed retainers, monitoring for wire breakage, and regular follow-up visits to check retention and overall dental health.
