Treatment approaches for temporomandibular joint disorders.

Temporomandibular joint (TMJ) disorders encompass a range of conditions affecting the jaw joint and surrounding muscles. Treatment typically begins with conservative approaches including self-care measures, physical therapy, and occlusal splint therapy. A stabilization splint or night guard worn during sleep helps reduce clenching and grinding forces on the joint. Pharmacological treatments include anti-inflammatory medications, muscle relaxants, and in some cases, low-dose tricyclic antidepressants for chronic pain management. Physical therapy techniques such as jaw exercises, manual therapy, ultrasound, and TENS (transcutaneous electrical nerve stimulation) help restore normal jaw function and reduce pain. When conservative treatments are insufficient, interventional options include arthrocentesis (joint lavage), corticosteroid injections, and botulinum toxin injections to relax overactive jaw muscles. Surgical intervention is reserved for the most severe cases and may include arthroscopy or open joint surgery. A multidisciplinary approach involving dentists, oral surgeons, and physical therapists often yields the best outcomes.
